Wags 2 Whiskers Veterinary Hospital is a paperless practice, however we have a small amount of paper forms and instructional handouts for our clients available on this page.

The forms that require your information are scanned into our computer system, shredded, and then placed in a recycle bin in our hospital.

New Clients

If you are a new client, you can print the form below and bring it in with you to expedite your first visit with our hospital. If you do not have access to a printer, or you wish to wait, you are welcome to fill this form out when you arrive. If you will not be bringing the form with you, we ask that you arrive at least 15 minutes earlier than your scheduled appoint time to fill out paperwork. We look forward to seeing you and your pets!

New Patients

If you are bringing a new patient to our hospital (one that we have never examined before), you will need to fill out the form below. Just as with the New Client Form, you may print this form ahead of time or wait to fill it out in our hospital.

Boarding

If you are going to be boarding your pet(s) with us, you will need to fill out the form below. Just as with the New Client Form, you may print this form ahead of time or wait to fill it out in our hospital.

Sick Patient – Drop Off

If you are bringing a patient to admit to our hospital who is not feeling well, you will need to fill out the form below. We use this form so the team will have a better idea of exactly the symptoms your pet is exhibiting after you leave him/her with us. Just as with the other forms, you may print this ahead of time or wait to fill it out in our hospital.

Surgical/Dental/Sedation Form

If you are bringing a patient to our hospital for either sedation for a procedure, dental cleaning, or surgery, you will need to fill out the form below. Just as with the other forms, you may print this form ahead of time or wait to fill it out in our hospital.

Surgery Discharge Sheets

If your pet has surgery in our hospital, we discharge your pet with these instructional handouts describing the necessary post-operative care we strongly advise. If for any reason, your discharge instructions are damaged or lost, you may print the appropriate sheet below.
